As for the Plaque, I use a small Brother P-Touch Label maker that takes 3/4 inch White on Black tape. The tape is then attached to a sheet of black plastic 1/16 inch thick (actually a 3 ring binder from an office supply store) and cut out to size. As most of my bases are horizontal in shape rather than vertical, it works out well for me, since I'm limited to the 3/4 inch height.
